Please return the signed claim form to dahlberg assurance brokers with documentation for your claim if available or other documentation of the loss, such as a receipt for the filed police report.
If you prefer to return the completed form to us electronically, you need to have either the Pro or the Business version of Adobe Acrobat Reader installed. If not, you can print out the completed form and return it to us by fax or regular post.
